We will use your information to respond to you, regarding the reason you contacted us. We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, e.g. to process or fulfill an order or to use our testing services as described in this privacy policy.

If you are a health professional, unless you ask us not to, we may contact you via email or telephone in the future to tell you about specials, new products or services, other company news, problems with your account or use of this website. If you are an individual who has used the testing services on this website, we will only contact you in response to your request or if there is a problem processing your testing.

This website collects potentially personally-identifying information like Internet Protocol (IP) addresses for logged in users and for users leaving comments on our blogs. We only disclose logged in user and commenter IP addresses under the same circumstances that we use and disclose personally-identifying information as described in this Privacy Policy, except that blog commenter IP addresses are visible and disclosed to the administrators of the blog where the comment was left.

Visitors to this website can always refuse to supply personally-identifying information, with the caveat that it may prevent them from engaging in, or affect the outcome of, certain website-related activities.

Wherever we collect sensitive information, such as registration data by health professionals to use our website or information provided as part of our testing services as described in this Privacy Policy, that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a lock icon in the address bar and looking for “https” at the beginning of the address of the Web page.

While we use encryption to protect sensitive information transmitted online, we also protect your information offline. Only employees who need the information to perform a specific job (for example, billing or customer service, or entering testing data (see Testing in this Privacy Policy) are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are kept in a secure environment.

Registration
In order for health professionals to establish an account and use the testing services available on this website, they must first complete the Registration form.  During registration the health professional is required to give certain information (such as name and email address). This information is used to contact them about the products/services on our site in which they expressed interest. The information they supply on the Registration form will be used by us to establish their account, generate a User Name and Password and send that to them along with other pertinent information regarding their requested use of our website and services provided.  The information will not be shared with any third parties.

Testing 
This website provides testing by the evaluation tool called The TMJ Scale.  A doctor, dentist, psychologist or other health professional may Register for an account and ask their patients to fill out the questions on the TMJ Scale.  This can be done using a paper and pencil version with the data subsequently entered on this website by the health professional’s office.  Or, the patient may be asked to directly enter their responses to the TMJ Scale on this website.

An individual may voluntarily self-test using The TMJ Scale on this website.

The TMJ Scale does not collect any directly personally identifiable information such as name, address, telephone number, social security number or email address.  It does collect demographic information such as age and gender, along with information related to the medical, physical, psychological, behavioral and social symptoms and problems that are being evaluated by the TMJ Scale.

The data collected by the TMJ Scale is analyzed by a proprietary and protected algorithm that produces a report on this website to the health professional or the individual, as the case may be.  The report may be saved and printed. The report presents the results of the analysis in the form of quantitative scores on certain scales and a written interpretation of those scores.

We do not share any information regarding the responses to, or resulting report of, the TMJ Scale with any third parties outside of its use as described in this Privacy Policy.

When a health professional (doctor, dentist, psychologist, etc.) administers the TMJ Scale to a patient they are doing that as part of the doctor-patient relationship and their medical practice and the information collected and derived is expected to be used according to the usual rules, guidelines, regulations and laws governing the medical record.  However, we have no knowledge or control over how that information is used or to whom it is made available by the health professional.

When an individual self-tests with the TMJ Scale they are responsible for the use of the resulting report and we have no knowledge or control over how that information is used or to whom it is made available.

We may use aggregated or individual results of using The TMJ Scale for research and publication in clinical journals in the fields of medicine and dentistry.  No directly personally identifying information is used in such publications.

 Orders
We use an outside credit card processing company to process on-line orders on this website for goods and services. This credit card company does not retain, share, store or use personally identifiable information for any secondary purposes beyond filling your order.

If we are notified by you or by the credit card processing company, or if we otherwise become aware of a problem processing an order, or if we notice something unusual about your order (such as an unexpected repeat order or a problem updating your account information based on your order), or if a refund is needed, we will use any and all of the information you have provided to our credit card processing company and voluntarily to us in order to contact you and correct the problem.
